Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that facilitates research into pragmatic trials. It gathers and distributes clean trial data, ratings and evaluations using PRECIS-2. This allows for diverse meta-epidemiological analyses to compare treatment effect estimates across trials of various levels of pragmatism.
Background
Pragmatic trials provide evidence from the real world that can be used to make clinical decisions. The term "pragmatic", however, is a word that is often used in contradiction and its definition and assessment need further clarification. Pragmatic trials must be designed to inform clinical practice and policy decisions, not to confirm an hypothesis that is based on a clinical or physiological basis. A pragmatic study should strive to be as close as it is to actual clinical practices that include recruitment of participants, setting, designing, delivery and implementation of interventions, determination and analysis results, as well as primary analyses. This is a key distinction from explanatory trials (as described by Schwartz and Lellouch1), which are designed to provide more thorough proof of a hypothesis.
Trials that are truly pragmatic should be careful not to blind patients or healthcare professionals as this could result in bias in the estimation of treatment effects. Pragmatic trials should also seek to recruit patients from a variety of health care settings to ensure that their findings are generalizable to the real world.
Furthermore, trials that are pragmatic must focus on outcomes that matter to patients, like the quality of life and functional recovery. This is especially important in trials that require invasive procedures or have potentially dangerous adverse effects. The CRASH trial29, for example was focused on functional outcomes to compare a two-page report with an electronic system for the monitoring of patients admitted to hospitals with chronic heart failure. In addition to these features pragmatic trials should also reduce the requirements for data collection and trial procedures to reduce costs and time commitments. Additionally pragmatic trials should strive to make their results as applicable to clinical practice as they can by making sure that their primary method of analysis is the intention-to-treat approach (as described in CONSORT extensions for pragmatic trials).
Despite these criteria however, a large number of RCTs with features that challenge pragmatism have been incorrectly self-labeled pragmatic and published in journals of all kinds. This can lead to misleading claims of pragmatism, and the term's use should be standardized. The creation of a PRECIS-2 tool that offers a standardized objective evaluation of the pragmatic characteristics is the first step.
Methods
In a practical study, the goal is to inform clinical or policy decisions by demonstrating how an intervention can be integrated into routine treatment in real-world situations. Explanatory trials test hypotheses concerning the cause-effect relationship within idealised environments. Consequently, pragmatic trials may have lower internal validity than explanatory trials, and could be more susceptible to bias in their design, conduct and analysis. Despite their limitations, pragmatic research can provide valuable information for decision-making within the context of healthcare.
The PRECIS-2 tool evaluates an RCT on 9 domains, with scores ranging from 1 to 5 (very pragmatist). In this study, the recruitment, organization, flexibility in delivery and follow-up domains were awarded high scores, however, the primary outcome and the method for missing data fell below the limit of practicality. This suggests that it is possible to design a trial with good pragmatic features without damaging the quality of its outcomes.
It is difficult to determine the degree of pragmatism that is present in a trial because pragmatism does not possess a specific attribute. Some aspects of a research study can be more pragmatic than other. Moreover, protocol or logistic modifications during the course of the trial may alter its score in pragmatism. Results
While the definition of pragmatism does not mean that trials must be 100% pragmatic, there are benefits to incorporating pragmatic components into clinical trials. These include:

Conclusions
In recent years, pragmatic trials are increasing in popularity in research because the value of real world evidence is increasingly recognized. They are randomized trials that compare real world alternatives to experimental treatments in development. They involve patient populations closer to those treated in regular care. This method can help overcome the limitations of observational research for example, the biases associated with the reliance on volunteers and the limited availability and coding variations in national registries.
Other advantages of pragmatic trials are the ability to utilize existing data sources, and a greater chance of detecting meaningful changes than traditional trials. However, these tests could be prone to limitations that undermine their validity and generalizability. The authors of the Pragmatic Free Trial Meta identified 48 RCTs self-labeled as pragmatist and published up to 2022. The PRECIS-2 tool was employed to determine the pragmatism of these trials. They found that 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or above) in at least one of these domains.
Trials with high pragmatism scores tend to have broader criteria for eligibility than conventional RCTs. They also have populations from various hospitals. The authors claim that these characteristics can help make pragmatic trials more meaningful and applicable to daily practice, but they do not necessarily guarantee that a trial using a pragmatic approach is free from bias. The pragmatism is not a definite characteristic and a test that doesn't have all the characteristics of an explanation study could still yield valid and useful outcomes.
